WHO NEEDS HOSPICE OR PALLIATIVE CARE?

Hospice and Palliative care are services that support the physical, emotional, and spiritual needs of individuals that have been diagnosed with a serious or chronic illness.

Hospice approaches our patients’ care as a team of multi-disciplinary professionals, the patient, and the family. Hospice includes the patient and family in developing a plan of care that coincides with the patient’s physician, who works closely with the Hospice team.

Living with a serious illness is hard. It can be confusing, and even scary. Our palliative care team can help. HPC - Palliative Care of Western Kentucky is a specialized service that supports the physical, emotional, and spiritual needs of individuals diagnosed with a serious or chronic illness.

Located on 10.8 acres in Owensboro, The Heartford House offers a beautiful, peaceful setting for patients and their families. Our facility was the first free-standing hospice inpatient facility in Western Kentucky. We’re located at 2914 Old Hartford Road in Owensboro.

Listed below are some different ways HPC - Hospice and Palliative Care of Western Kentucky gives back to the community.

Camp Courage is a free one-day bereavement program for children available to the entire community. Held twice a year for children who have experienced the death of a loved one, Camp Courage is an interactive program that empowers children to confront their grief as a normal, individualized response to loss. Led by our trained bereavement professionals and volunteers, the campers engage in activities that help them to develop fundamental coping skills to heal and live well during the grieving process.

Many of our patients have end-of-life wishes they want to fulfill. In 2008, we created our WISH Program to help those wishes come true! Through our WISH Program, patients and families have gone on weekend trips, trips to visit loved ones, and fishing trips, to name a few. Some wishes are big. Some are small, but each wish is unique and special to our patients.

Grief naturally sets in anytime we experience loss. There are numerous types of loss: divorce, loss of a pet, loss of a job, and loss of independence. One of the most challenging losses we inevitably face is the death of a loved one. If you or someone you know is in the wake of experiencing any loss, our Grief Support Team can help. CARING BEAR SERVICES

Children often communicate their feelings through behavior. Misconduct or abnormal demeanor may be the child’s way of saying, “I’m scared, what is happening to Grandmother, or I don’t understand what is going on.” These cuddly keepsakes not only provide security and companionship but also share a personal story about the bear and the impending loss of his loved one, giving the child a gentle outlet to relate to their situation.

CAMP ERIN

Camp Erin Western Kentucky is a free overnight bereavement camp for youth who are grieving the death of significant person in their lives. Children and teens ages 6 to 17 attend a weekend camp experience that combines grief education and emotional support with fun, traditional camp activities at the Gasper River Retreat Center in Bowling Green, KY. Led by bereavement professionals and caring volunteers, campers are provided a safe environment to explore their grief, learn essential coping skills, and make friends with peers who are also grieving.
